If I remember correctly, they are rated from 1 to 4, 4 being the worst. Most insurances will not pay until ones vision is affected greatly and rated at least a 3. That is how it was explained when I had one eye done 8 years ago.
I had trouble with mine. But now years later all is good. Held off on making a comment due to this fact. Basically went from 20/15 to 20/70 after the operation resulted in a astigmatism to the eye.
