The least expensive in general system would be a single payer. Just one plan for everyone. Covering cradle to grave ones total healthcare needs. This would eliminate the complexities and redundant paperwork we have now. But then we would have leaders who truly care about the citizens health.
Of course the industry would hate this approach. Part of the cost saving would be all the jobs lost because of the streamlining.

So just like tax reform will not be a complete one, because of all the jobs lost to those who get paid to do others taxes. Good meaningful reform to healthcare will never happen as long as lobbyist of the industry pull the strings.
