Should basic preventative, palliative, and curative health care in the USA be a right or a privilege? It is well-known that hospital emergency room services are the least cost-effective and should be avoided to minimize loss over return.
Granted a doctor's services are by need a commodity so that the provider can make a living, tho' the service itself can be provided by a shared liability ala common insurance and the way it works, the broadest participation spreads the thinnest ongoing subscription cost over the largest user base. So, how to implement this controlling costs of materials and services without overloading the process with excessive administration costs?
IIRC Kaiser established a company branch to accomplish this and did quite well. The military staffs (or used to) it's medical services by paying for schooling with a commitment for service in uniform for a given period at military officer's pay scale.
Dealing with any company answerable to stock-holders instead of it's staff and customers seems a non-starter for efficiency and motivation due to their primary goal of maximizing profits.
AFAIK Kaiser was the only private enterprise marketplace solution that worked well due to the internalization of oversight and services.
